New Patient Registration

Please note: our list is temporarily closed to local patients that already have a GP in the area due to local GP sustainability issues.

Our list is open to all patients moving in to the practice area.

You can register by bringing along your NHS medical card or completing the forms available from reception or downloadable below. Change of Personal Details

If you change name, address or telephone number, please come into the surgery and fill in a "Change of Details" form together with proof of your new address or any proof of other changes. If you move without telling us we might write to, visit or telephone the wrong house. It is particularly important that we have our correct telephone number in case we need to contact you urgently.

If you move outside the practice area you will need to find a doctor in your new area. There are instructions on what to do when you move to a new area on the back of your NHS medical card.

Remember to tell other agencies about your change of details too - Hospital, Dentist etc.
