New Patients

New patient registration

We welcome new patients to register with our practice. As it often takes some time for records to be forwarded from your former practice all newly registered patients will be asked to complete a New Patient Health Questionnaire and registration form. You will also require your NHS number.

The practice will require proof of identity which is photographic and also proof of residence within the last 6 weeks (utility bill, bank statement, etc). You will need to bring your identification, completed registration form and new patient questionnaire with you to your appointment. Alternatively, you may use the online form below to register.

How to register

Alternatively, you can visit the Surgery and collect the appropriate forms. It may be necessary to be seen by the healthcare assistant, who will complete your medical history questionnaire.

Updating your contact details

There may be times where the Practice may need to contact you by telephone or letter.

If you change name, address or telephone number, please come into the surgery and fill in a “Change of Details” form together with proof of your new address or any proof of other changes.

If you move without telling us we might write to, visit or telephone the wrong house. It is particularly important that we have our correct telephone number in case we need to contact you urgently.

Please be aware if you have changed your name, we require a copy of the legal document or marriage certificate.

New Patient Registration

DURING CORONAVIRUS (COVID-19) PERIOD

We welcome new patients.

We require all new patients to complete a registration form (GMS1W), and a health questionnaire (below) which will provide useful information whilst we wait for your medical records to arrive from your previous doctor. Both forms can be found below.  We will accept two forms of identification that prove you live within the area, photo and one with proof of address. Your NHS number is also required to register, this can be obtained from your previous surgery. Please also supply a copy of any repeat medications from your previous GP to ensure we have proof of medication. 

Please complete and attach using the online forms below. We will accept a photograph of the evidence; if you can not access a printer/scanner. Please note if you do not complete all information required or attach proof of address this will delay your registration.

For patient safety and confidentiality; please note by returning a completed form you are signifying you are who you say you are and all information is correct to the best of your knowledge. This will mean you also give permission for us to set you up as a New Permanent Patient at our Practice.
